Dear Applicant,

Thank you for your interest in Canyon Creek Sports Camp! Please take your time to fill out the following application completely as soon as possible. You can either complete it online or download the pdf file and return by mail or fax to our Canyon Creek Sports Camp WINTER OFFICE (listed above).

When filling out the application, please keep the following in mind:

• You will need to have 2 personal reference forms completed. We recommend getting these forms to your references as soon as possible as this portion of the application often takes the most time to be returned. Please let your references know that they may either return the forms to you to have them included with your application or they may mail or fax them directly to camp.
• Please fill out the application completely and be sure to include the most current information.
• If possible, please include a photo of yourself with your completed application so we may keep it on file with your information.

Once we receive your completed application, we will contact you to set up an interview - either in person, if you are a local applicant, or by phone if you are an out-of-state or international applicant.
