Dear Applicant,

Thank you for your interest in Canyon Creek Sports Camp! The following Assistant Counselor application is for applicants under 18 years of age. Please take your time to fill out the following application completely as soon as possible. You can either complete it online or download the pdf file and return by mail or fax to our Canyon Creek Sports Camp WINTER OFFICE (listed above).

When filling out the application, please keep the following in mind:

• The Canyon Creek Sports Camp Assistant Counselor program is very limited, so please take your time and thoughtfully fill out the application in its entirety. Partial or incomplete applications will not be considered until completion.
• You will need to have 2 personal reference forms completed. We recommend getting these forms to your references as soon as possible as this portion of the application often takes the most time to be returned. Please let your references know that they may either return the forms to you to have them included with your application or they may mail or fax them directly to our WINTER CAMP OFFICE.
• Be sure to include the most current information. If you are unsure about any part of the application or have any questions, please do not hesitate to call either camp office for assistance.
• If possible, please include a photo of yourself with your completed application so we may keep it on file with your information.

Once we receive your completed application, we will contact you to set up an interview - either in person, if you are a local applicant, or by phone if you are an out-of-state or international applicant.
